Change in feedback template

We need changes for the feedback template. I have indicated numbers on the screenshot.
1: Possibility to remove the placeholder: since the field already has a tag, the placeholder is simply repeating.
2: Others: should be other (others is only for people, not objects/issues)
- email should be email address (we make a difference between 'email' (the message) and email address.

I've received feedback from internal users that the option "Others" is awkward; should be "Other," but ideally we would like to be able to edit these feedback options or create our own.
